Our home health care agency implements programs and services to provide comprehensive, quality home health care in the patient’s home.
We address the patient’s unique psychological, safety, self-esteem, and self-actualization needs in a patient plan of home health care. All Home and Community-Based Services (HCBS) are for persons with disabilities and persons age 65 and older in any county in the state, subject to the standards of Minnesota Statutes, Chapter 245A and Minnesota Statutes, Chapter 245D.
Spectrum Home Health Care, LLC serves as a resource for the patient, the patient’s family, and the community. We provide health care education for the patient, the patient’s family, and the community. The agency will directly provide or arrange for skilled nursing services provided by registered nurses and licensed practical/vocational nurses (if hired by the Agency), and paraprofessional services provided by home health aides, companion, and direct support workers. Similarly, services provided by physical, occupational, and speech therapist, and medical social workers will be provided to the extent that such professionals are either hired by the agency or contracted for by the agency.
